To self-certify, you’ll need to: 1. Log in to your Releaf account 2. Click the ‘additional information required for your triage’ alert on your dashboard 3. Open the form and upload proof of previously tried treatments (e.g. prescriptions, over-the-counter medicines, or therapy invoices) 4. Add any relevant notes, then submit the form to complete your self-declaration This lets our world-class clinical team begin the triage process sooner. Your NHS Summary Care Record will still be needed before a prescription is released, but your consultation doesn’t have to be delayed.

You can request an invoice for any of your past or present prescriptions from your Patient Dashboard. Select the ‘Prescriptions’ tab to view all past and present prescriptions and select the prescription you would like the invoice for. Once you have selected the prescription for which you would like an invoice, click ‘Request an invoice' and the invoice will be generated for you.
